There's a lot of changes to the gameplay, such as removing grenades, Tier 2 weapons, a tank and witch spawn on every map, a new scoring system that factors HP, and a lot more. Each config is different, for example in confogl you'll have 4 medkits per map, but in zonemod medkits are removed completely and every survivor has a set of pills instead.
